BJC Home Care Services offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs as well as the Lifeline emergency response system. The BJC Home Care Services Supportive Care staff offer patients with life-limiting illness an opportunity to ease the transition from curative treatment to Hospice care through a unique program. Supportive Care offers a bridge between curative and terminal care. It is a multidisciplinary approach, holistic in nature, centered on the needs of the patient and patient's family, that provides coordinated medical, emotional and spiritual care.
Overview
Role Purpose Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source.
