BJC Behavioral Health is a community health center that provides and coordinates behavioral health services for more than 6,000 seriously mentally ill adults and seriously emotionally disturbed children in St. Louis City, St. Louis County, St. François, Iron and Washington counties.  As an Administrative Agent of the Missouri Department of Mental Health (DMH) serves as a major point of entry for people eligible for mental health services funded by DMH and is responsible for serving as gatekeeper to the public mental health system.  BJC Behavioral Health also administers the BJC Employee Assistance Program ad Behavioral Health Partners, an administrative services organization.


Role Purpose

This position focuses on population health management through use of Health Information Technology. Objectives are achieved by use of population health data to champion, develop and enhance wellness initiatives and provide support to the healthcare team in improving the health outcomes of their patients.


  • Consult with and support Community Support Specialists about identified health conditions of their clients. Facilitate health education and training for clients/staff/community on medical diseases, chronic illnesses, health coaching, healthy lifestyle choices, medications and medication adherence.
  • Assist organization in developing and enhancing wellness initiatives. Champion preventive care efforts and healthy lifestyle changes, including tobacco and other substance cessation, nutrition, physical activity, stress management, and chronic disease self-management.
  • Use Health Information Technology (HIT) tools for population health management; manage caseload through follow-up with healthcare providers on health and medication alerts.
  • Coordinate with Community Support Specialists and hospital staff regarding hospital admissions and discharges, and assure hospital discharge follow-up and medication reconciliation.
  • Track that all required assessments and screenings are completed for each client and follow-up as necessary. Assist in identifying healthcare goals and developing treatment plan with clients.
  • BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
