Overview


Role Purpose

Provides social work liaison, including referral and direct services, for patients families, physicians, staff, agencies and others involved in social work services. Coordinates administrative office management for the Manager and the Department of Clinical Social Work and Chaplaincy.


Responsibilities

  • Secures providers of care for patient post-discharge needs.
  • Provides support to the case management program.
