BJC Home Care Services offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs as well as the Lifeline emergency response system. BJC Home Care Services provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country.

The Central Referral Center (CRC) for BJC Home Care Services is where the patient referral process starts. There are Intake Coordinators in each BJC hospital who coordinate services for those patients that need home care. The patients who choose BJC Home Care Services are then channeled through the CRC. The CRC also consists of nurses who take physician office referrals for patients not currently hospitalized. There is someone on-call 24/7 for the CRC to facilitate home care coordination for patients in emergency rooms, etc.


Overview


Role Purpose

Clinical Manager is responsible for managing in coordination with HSO case management the safe and effective transition of patients onto Home Care. The position is also responsible for identifying, initiating, developing and managing business relationships with hospitals, physicians, post-acute care settings, and managed care payors. This position coordinates and executes objectives identified by the organization to reach both short term and long range growth plans. This position ensures department practices support agency and state and federal regulations, ensuring high reliability of clinical and financial processes.


Responsibilities

  • Recommends, develops, updates strategic business plans to support organizational practices and goals. Develops and implements effective sales strategies by establishing goals, monitoring performance and creating incentive program. Assists senior leadership with setting budget and revenue goals, product development, identifying new business opportunities and strategies.
  • Participates in system care coordination, including utilization and quality initiatives. Works with both internal and external health care providers to achieve optimal patient mix and ensure safe transitions of care. Establishes and maintains key client relationships.
  • Ensures the highest quality of care in compliance with local, state, and federal regulations. Ensures patients receive the quality care by communication, coordination and documentation of clinical, demographic, and payor information.
  • Manages individual(s) including but not limited to: hires, trains, assigns work, manages & evaluates performance, conducts professional development plans. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.
  • Develops and adheres to departmental staffing, revenue and/or expense budgets. Responds to changes in the business which may affect the ability to achieve the budget goals.
  • BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
