Boone Hospital Center is a 394-bed full service hospital located in Columbia, MO. It is a regional referral center located in the center of the state. The hospital provides progressive healthcare programs, services, and technology to people in 26 mid-Missouri counties. Although the hospital is full service, areas in which Boone Hospital Center excels are cardiology, neurology, oncology, surgical, obstetrical services, and numerous other specialties. The hospital maintains a 24-hour emergency center with hospital-based ambulance service and a helipad for incoming emergency air transportation.  Recognizing its excellence in nursing care, Boone Hospital Center has been certified as a "Magnet Hospital" by the American Nurses Credentialing Center.
 

The Boone Hospital Center Weight Loss Surgery program offers a comprehensive multi-disciplinary team for people who are considering Weight Loss Surgery such as LAP-BAND or Gastric Sleeve.  The Bariatric Coordinator leads the team in providing in depth education to each patient regarding lifestyle changes that are necessary for successful outcomes.  Public educational seminars along with monthly support group meetings are offered.  Each patient is guided through the insurance requirements and this program serves as a reference to inpatient care areas that are dealing with Bariatric patients.  Very detailed pre and post-surgery data is tracked and analyzed to ensure quality outcomes.


Overview


Role Purpose

Coordinates day-to-day operation of the Bariatric Program. Optimizes service, treatment, and education is provided to all customers and patients across the continuum of care. Responsible for coordination of patient visits, patient education, prepares patient/family for procedures and testing in collaboration with the healthcare team.


Responsibilities

  • Facilitates patient care by assessing and evaluating patient's physical, psychosocial and emotional needs.
  • Coordinates the delivery of safe and appropriate patient care governed by evidence-based clinical practice.
  • Monitors care and data to optimize patient outcomes.
  • Facilitates the education of patients and families; serves as liaison for the care team.
  • .Ensures actions meet and support the overall goals of the Bariatric Program

  • Minimum Requirements

     

    Degree

  • Bachelor's Degree
  • - Nursing/Nutrition/related

     

    Experience

  • 2-5 years
  •  

    Supervisor Experience

  • < 2 years
  •  

    Licenses & Certifications

  • Licensed Dietician or RN
