Boone Hospital Center is a 394-bed full service hospital located in Columbia, MO. It is a regional referral center located in the center of the state. The hospital provides progressive healthcare programs, services, and technology to people in 26 mid-Missouri counties. Although the hospital is full service, areas in which Boone Hospital Center excels are cardiology, neurology, oncology, surgical, obstetrical services, and numerous other specialties. The hospital maintains a 24-hour emergency center with hospital-based ambulance service and a helipad for incoming emergency air transportation.  Recognizing its excellence in nursing care, Boone Hospital Center has been certified as a "Magnet Hospital" by the American Nurses Credentialing Center.

The Supportive Care Program at Boone Hospital Center has the primary mission of relieving the pain, suffering and symptoms of those who cannot be cured. It offers direct patient care as well as emotional support, education and resources for the patient and family. The Supportive Care team is an active member of the multi-disciplinary care team and is available for patients throughout the hospital. The program also seeks to develop a larger local community knowledgeable about palliative medicine and offers an invitation to all hospital staff to support and be involved in the program. A final goal is to connect with those outside of mid-Missouri to expand access to palliative care to all people.

Role Purpose

Responsible for assessing and coordinating the physical, psychosocial and spiritual needs of the patient and family at end-of-life, as well as pain and symptom management at all stages of the chronic disease process. Develops and coordinates the palliative care plan with physicians and nurses, along with services such as Care Coordination, Pastoral Care, Home Health and Hospice care.


  • Consults with physicians, patients, families and the Palliative Care Team to develop and modify individualized plans of care.
  • Provides ongoing case management for seriously ill and end of life patients.
  • Provides educational opportunities for physicians, nurses, and other allied health care providers regarding palliative care and advanced directives, and integrates palliative care topics into community education programs.
