Role Purpose

Plans, coordinates and leads activities of the program. Supports the operational functions of the department as related to: scheduling, staff communication, data collection, report preparation, regulatory readiness compliance, and communication regarding department. Assesses, plans, evaluates and documents the nursing process of program’s patients. Takes a lead role in coordination with physicians and education of staff and physicians regarding supportive care and end of life care. Works collaboratively with other members of the leadership team to lead staff in clinical and service excellence.


Responsibilities

  • Manages individual(s) including but not limited to: hires, trains, assigns work, manages & evaluates performance, conducts professional development plans. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.
  • Develops and adheres to departmental staffing, revenue and/or expense budgets. Responds to changes in the business which may affect the ability to achieve the budget goals.
  • Consults with physicians, patients, families and the Palliative Care Team to develop and modify individualized plans of care.
  • Provides ongoing case management for seriously ill and end of life patients.
  • Provides educational opportunities for physicians, nurses, and other allied health care providers regarding palliative care and advanced directives.
