BJC Home Care Services offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. The Metro St. Louis Intermittent Home Care Department of BJC Home Care Services provides home visits to patients in the metropolitan St. Louis area and several nearby counties, with 24 hour on-call home care nursing supervision. Role Purpose

The Quality Assurance and Performance Improvement Coordinator (QAPI) is responsible for coordinating and/or implementing the collection of quality and compliance-related data, which includes but is not limited to review of patient records and/or other clinically-related logs, identification and trending of deficiencies, analysis of trended data, and instruction and in-service to designated staff. Assists in monitoring performance improvement activities of the staff, as well as participates in orientation. education, and competency-assessment as assigned. Position requires an in-depth knowledge of the Medicare Conditions of Participation for home health care, Missouri regulations, and accreditation standards as applicable to home health, in addition to knowledge of home care population-specific (e.g., age or care specific) requirements


Responsibilities

  • Performance Improvement - Collects, aggregates, and trends data in acceptable reporting format and within designated timeframes. Evaluates data, identifies problem areas, and participates in problem resolution following the established quality improvement process. - Be knowledgeable in data collection, data analysis methodology, and performance improvement methods needed to support and lead QAPI. - Lead performance improvement projects and provide education and coaching in order to build needed skills in others to lead PIPs. - Identify resources and tools to support improvement efforts. - Identify and manage potential barriers to the success of QAPI efforts. - Assist teams working on QAPI efforts to identify any unintended consequences of changes that are put in place in order to avoid negative impact on overall performance.
  • Collaboration/Coordination - Collaborates with administration, patient care services, leadership, shared leadership councils, staff and key department and BJC system personnel to develop education plans and implementation patient care and regulatory requirements. - Communicate regularly on the progress of QAPI work to leadership and staff, including the progress of performance improvement projects - Work with the administrator to secure resources for performance improvement work. Manage the financial, material and human resources allocated to carry out QAPI work. - Coordinate with directors/managers to ensure QAPI efforts are integrated into all operations and departments, including staff responsibilities. - Identify opportunities for improvement through analysis of data, observation of operations and consultation with leadership, staff, residents, families, and other stakeholders. - Maintains continuous mandatory regulatory readiness for accrediting agencies. Coordinates and manages external regulator inspections for all appropriate accrediting bodies.
  • Design/Organization - Organize and facilitate the quality committee and its meetings by guiding discussion around performance measures, and prioritizing and developing quality efforts. - Create processes to design and charter performance improvement projects that includes establishment of indicators of success. - Design a process for and conduct an evaluation of QAPI efforts (at least annually). Facilitate a discussion within the quality committee for this annual review. - Develops, coordinates, and implements additional staff training identified through quality/regulatory compliance activities as recommended by management team.
  • Coaching/Mentor/Education - Provides consultative lead on assigned specific functions, process, or committee, related to staff/patient education, staff/patient safety, regulations, quality, etc. - Coach staff, management, and practitioners on how to problem solve, including how to analyze causes and contributing factors. - Identify staff training needs to support QAPI. - Encourage staff member involvement in QAPI.
